READ BEFORE SIGNING

RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in More Keepers Group Training, Camps, Clinics or Individual Training with More Keepers LLC, Coach Marcelo Moreira or his assistant coaches, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participants enrolling in More Keepers programming understand he/she is using The
   Athletic Campus, Glacier Ridge Sports Park, Epic Sports Center, Nazareth University, Christian Central Academy and any facility
   authorized for use be More Keepers at his/her own risk. More Keepers, LLC and its
   owners, employees, workers, and agents shall not be liable for any damage whatsoever
   arising from any personal injury or property loss sustained by participant with his/her
   family in or about any programs on the premises, or any associated facility utilized for
   Goalkeeper training/camps or any event. Participants and parents assume full
   responsibility for all injuries and damages which occur in or about any programs on the
   premises. He/she does hereby fully and forever release discharged hold harmless More
   Keepers, LLC, all associated facilities and its owners, employees, workers, and agents
   from any and all claims, demands, famages or rights of action, present or future
   resulting from any person’s participation in any program. This agreement applies to 1)
   personal injury (including death) from accidents or illnesses arising directly or indirectly
   from participation in activities directed, suggested, or planned by More Keepers
   including but not limited to, organized activities, classes, instruction, observation, related
   activities in a non-supervised setting, and use of facilities, premises or equipment; and
   to 2) any and all claims resulting from the damage to, loss of, or theft of property. I
   further agree to indemnify and hold harmless and defend More Keepers, LLC, it’s agent,
   employees, directors, officers and affiliates, from any claims resulting from the injuries,
   damages, illnesses and losses sustained by me or caused by me arising out of the
   association, connection with or in any way with my participation in any fitness and
   conditioning activities conducted with More Keepers, LLC. I further state that I am in
   proper physical and mental condition to participate in More Keepers trainings. Injuries
   may result in the participation in More Keepers or its related activities, such as, but not
   limited to broken bones, torn ligaments, pulled muscles, bruises, etc. I am willing and
   voluntarily assume all risks in my training with More Keepers.
   I consent, as parent or guardian/participant do hereby grant authority to the staff of
   More Keepers to render a judgment concerning medical assistance or hospital care in
   the event of an accident or illness during my absence.
   I understand that the only insurance I am covered under, if any, will be my own and I
   understand that I have had the opportunity to secure this prior to participation.

2. FEES:
   Training fees are non-refundable/non-transferable. No fee payment or parts thereof will
   be refunded or forgiven because of player injuries, absences, withdrawals, or early
   release from our programming. Please note that refunds or credits will not be issued
   due to a player’s academic performance or “workload”. The only exception to this policy
   may be if the player provides documented proof of a season ending injury or due to a
   parent’s job relocation.

   In the event that any programming has to be cancelled due to environmental factors
   outside of More Keepers control, i.e. weather, health pandemic, Acts of God, etc. More
   Keepers will make their best effort to make up any lost playing time. Any credit issued
   would be at the discretion of Management.

3. Marketing, Website, Social Media:
   I hereby authorize More Keepers LLC and its assigns to utilize any and all photographs,
   pictures or other likeness of me or anyone assigned guardianship to me, as they deem
   appropriate in its promotional materials.
   I have read this release of liability and assumption of risk agreement, full
   understand its terms, understand that I have given up substantial rights by
   signing it, and sign it freely and voluntarily without any inducement.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
